1. Ingest

Prescient connects to your EHR over HL7 FHIR to continuously ingest vitals, labs, medication administration, and clinical notes — without requiring a separate system of record or duplicate data entry from clinical staff.

2. Score

Every patient is continuously scored across the relevant risk pillars — sepsis and deterioration, advanced illness, and AMR — and reported on a standardized four-band scale so risk is comparable across units and conditions.

  • Low (0–39) · Moderate (40–64) · High (65–84) · Critical (85–100)
  • Scores update as new data arrives, not on a fixed interval
  • Each score is paired with the contributing factors behind it

3. Alert

When a patient's risk band changes, the alert routes to the clinician and team already responsible for that patient — inside the workflow they use today, not a separate dashboard to check.

  • Alerts explain why the score changed, not just that it did
  • Designed to reduce alert fatigue by routing only meaningful band changes
